Prenatal Care Coordination
The primary goal of Prenatal Care Coordination is early entrance and continued participation in prenatal and postpartum care to reduce low birthweight, preterm labor, and infant mortality.
Prenatal Care Coordinators provide support services and referral to health and human services for pregnant women and their families at risk for poor pregnancy outcomes due to physical, social, psychological, financial, or environmental factors.
IPN works closely with state Medicaid officials, Indiana State Department of Health, MCH Services and representatives from the three Medicaid Managed Care organizations to help streamline care coordination processes and provide ongoing communication and education to prenatal care coordinators throughout the state.
